FT telemed providers maintain a minimum of 5 licenses in states where Theoria actively provides care. The provider manages the clinical care of the population at the assigned SNFs. The provider monitors message boards, answers calls, and communicates with assigned SNFs during shifts. Provider will provide high-quality, patient-centered, comprehensive primary care services to patients residing in SNFs. The provider will be available for phone/secure messaging consultations with facility nursing staff.
